Personal Care
• Personal care: Assisting with tasks such as bathing, dressing, grooming, and toileting.
• Medication management: Prompting medication from blister pack, monitoring supply.
• Mobility assistance: Helping clients move around safely, using mobility aids if necessary.•
Meal preparation: Preparing nutritious meals according to dietary, religious or cultural requirements and preferences. Offering hot and cold drinks

Sitting Calls
• Needing a break from palliative care during the night hours.
• Taking a well-deserved break to catch up on some much needed sleep.
• Needing a short respite service for their loved ones.
• Needing to have some quality time.
• Needing reassurance that the cared for are provided with good quality care and individual support and company in their short absence.

Social Inclusion
Our experienced staff will work closely with you to develop different packages of activities. We work hard to identify skills and areas of development, making sure everyone who attends our activities has a fulfilling experience. These will be meaningful, stimulating, enjoyable whilst enabling independence as well as personal growth through identified learning needs and areas of interest. We develop activities and learning modules based on feedback from our customers according to what you enjoy doing.
Our activity programme includes sensory based activities, this is relaxing and calming for customers with complex needs. We also support various activities in the local community such as;

• Summer walks along water canals
• Cycling trips
• Sporting activities
• Sunday visits to car boots collection
• Fishing trips
• Holiday trips to seaside

End-of-life Care
We provide a range of services to those with a terminal diagnosis. Hopeful Health Care is there to provide physical, emotional, social, and spiritual support for service users and their families during defining moments of life. We offer this service from the time of diagnosis through end-of-life.

Respite Care
We help primary caregivers and families get some relief by providing temporary care services to their loved ones. Allowing them time to recharge whilst ensuring that the individual receiving care is looked after by trained professionals in a safe environment.
We strive to:
• Promote the mental health and wellbeing of our carers and the individual being cared for.
• Prevent burnouts and provide the opportunity for a break to family members or carers.
• Offer a person centered care, placing our service users at the center of our services.

Supported living for Adults
Our Supported Living services help people to live independently and manage their own tenancies, with support designed around their needs and goals. We aim to promote choice and personalise our services to the needs of our service users. We support adults with learning disabilities, mental health needs, dementia, post hospital discharge and other complex care needs.
Our adults service works to provide a calming and therapeutic environment offering support and accommodation for adults who are living with Learning Difficulties.
We work closely with service users currently living within secured units who have been deemed ready to continue their recovery journey and transition into the community.
Our service is outcome based with an emphasis on Service Users eventually being able to secure their own tenancies with minimal support where possible.
